Glad you asked. Click on the red button (New Patients) on our main page. This will download a form you can fill out to get the process started. Once this is filled out call our office at 360.378.1338 to get scheduled. Right now we’re only taking patients into our traditional side of the practice (meaning we bill insurance or Medicare/Medicaid). We’ll announce enrollment in Direct Primary Care soon; you can learn more about that option here on our website.
