Eventide 2.0 – details coming soon
Patients can expect a letter with details about our new care model in the next few weeks. This rollout next year will be for patients of ALL AGES. Once the letter has gone out we’ll post more here and in the local papers. Our pre-enrollment list is already starting to fill.
